Actinomycosis is a chronic, slowly progressive and suppurative infection, caused by actinomyces israeli and actinomyces gerenecseriae. limb involvement is not usual in this infection and limb amputation has been reported in a few cases. Here, we report a case of resistant right hand actinomycotic osteomyelitis of distal radius, carpal, and metacarpal bones in a 38-year-old woman, treated with various antibiotics, several debridement sessions, and amputation, finally. Imaging and pathological information are discussed and different medical and surgical treatment approaches are included. Taking a good history and timely diagnosis can lead to treating actinomycosis through medical and surgical treatments, if needed, before the catastrophic outcomes and amputation.
