Velopharyngeal Insufficiency Caused by Diphtheria: a case report presentation

    A 37-year-old Afghan man presented to the otorhinolaryngology clinic at Shahid Sadoughi Hospital in Yazd, Iran, complaining hypernasal speech, nasal regurgitation, and aspiration into the nasal cavity while eating. He had no history of underlying diseases. The patient had a history of fever, sore throat, and suppurative pharyngitis with severe dysphagia 20 days ago. In fiber-optics evaluation velopharyngeal insufficiency circumferential pattern was found with normal laryngeal activity. On neurologic examinations, cerebrospinal fluid analysis indicated an increase in protein levels and the electromyography/ nerve conduction velocity reported identified subacute acquired uniform sensorimotor demyelination polyneuropathy. He had no history of any vaccine injections since birth and was an afghan person, so diphtheria antitoxin serology was assessed in the patient, and the results showed that the level of the Diphtheria Ab (IgG) was very high and diagnosis of diphtheria was made. After three months of follow-up, the disease was completely cured and the patient was recovered.
